What forms of payment do you accept?
AMEX, VISA, MASTERCARD, or personal check with a credit card on file. LifeSpan pharmacy only accept AMEX, VISA, or MASTERCARD.
Do you have a payment plan?
No, we require payment in full on the day of your exam by one of our accepted forms of payment.
Do you take insurance?
No we do not. We are a private pay facility. We do not accept insurance reimbursement as payment. Payment is due in full on the day of your appointment. We will file our charges on your insurance plan as a courtesy to you. Any reimbursement received is forwarded directly to you.
If you file for me how much coverage should I expect?
Your reimbursement depends on the insurance company you use as well as the type of plan you have. If you would like information about our average reimbursement rate with specific companies, please contact our office.
What is your cancellation policy?
We will assess a cancellation fee for any appointment cancelled within 24 business hours of the original appointment time.
